Acceptance of Terms

Welcome to eHormone & Wellness Center, operated by IHC LLC ("we," "us," or "our"). By accessing our website at www.e-hwc.com, scheduling a consultation, using our patient portal, or receiving any services from us, you agree to be bound by these Terms of Service ("Terms").

Please read these Terms carefully before using our services. If you do not agree with any part of these Terms, you must not use our website or services.

Services Overview

eHormone & Wellness Center provides telehealth-based health and wellness services, including but not limited to:

• Hormone replacement therapy (TRT and BHRT) for men and women
• Medical weight loss programs, including GLP-1 therapies
• Peptide therapy protocols
• Sexual health and dysfunction treatments
• Thyroid optimization and related hormonal support
• Lab review, medication management, and ongoing care coordination

We operate primarily via telehealth and serve licensed patients in the states of Texas and Arizona. In-person visits are available for Texas patients only. All services are provided by or under the supervision of licensed medical providers.

Eligibility

To use our services, you must:

• Be at least 21 years of age
• Be a legal resident of and physically located in Texas or Arizona at the time services are rendered
• Have the legal capacity to enter into a binding agreement
• Not be enrolled in Medicare or Medicaid
• Provide accurate, current, and complete information about yourself and your health history

We reserve the right to refuse service to anyone who does not meet these eligibility criteria or who we determine, at our sole discretion, is not an appropriate candidate for our services.

Telehealth Services

By using our telehealth services, you acknowledge and agree to the following:

• Telehealth services involve the delivery of healthcare using electronic communications, including video conferencing, secure messaging, and phone consultations
• Telehealth is not a substitute for in-person emergency care; in the event of a medical emergency, you should call 911 or go to the nearest emergency room immediately
• Technical difficulties may occur and may interrupt or prevent delivery of care; we are not liable for service interruptions caused by factors outside our control
• You are responsible for ensuring you have a private, secure location and adequate technology (internet connection, device, etc.) for telehealth appointments
• Telehealth consultations are conducted by licensed providers, but the provider-patient relationship is subject to the limitations inherent in remote care delivery
• All telehealth services must occur while you are physically located within a state where we are licensed to provide care (currently Texas and Arizona)

Account & Registration

To access certain services, you will be required to create a patient account through our patient portal (ChARM Health). You agree to:

• Provide accurate, current, and complete registration information
• Maintain and promptly update your information to keep it accurate and complete
• Maintain the confidentiality of your login credentials and not share them with any third party
• Notify us immediately if you suspect unauthorized access to your account
• Accept responsibility for all activities that occur under your account

We reserve the right to suspend or terminate your account if any information you provide is inaccurate, misleading, or incomplete, or if you violate these Terms.

Medical Disclaimer

The information provided on our website is for general informational and educational purposes only and does not constitute medical advice. It is not intended to be a substitute for professional medical advice, diagnosis, or treatment.

Our licensed providers will conduct a proper medical evaluation before recommending or prescribing any treatment. Individual results vary and are not guaranteed. The testimonials and results described on our website are not necessarily typical and may not reflect your individual experience.

Our services are not appropriate for every individual. Your provider will advise you if our services are not appropriate for your specific circumstances.

Payment & Fees

By engaging our services, you agree to pay all applicable fees as disclosed at the time of scheduling or enrollment. Key payment terms include:

• Initial Medical Consultation: $150 (may be waived if current labs are on file from within the last 3 months)
• Follow-Up Lab Review: $250
• Monthly Membership Programs: Billed on a recurring monthly basis via auto-pay. Current pricing is available on our Pricing page
• Annual Memberships: Billed annually and are non-refundable
• Prior Authorization Requests: $99 per request (non-refundable)
• Laboratory Testing: Not included in membership fees; billed separately through your insurance or at cash-pay rates

All payments are due at the time of service unless otherwise agreed in writing. We accept major credit cards and other payment methods as indicated at the time of payment. We use secure, third-party payment processors and do not store full payment card information.

Fees are subject to change with reasonable notice. Continued use of services after a fee change constitutes your acceptance of the updated fees.

Cancellation Policy

Monthly Memberships: You may cancel your monthly membership at any time by providing a minimum of 14 days' written notice prior to your next billing date. Cancellation requests must be submitted in writing to info@e-hwc.com. Verbal cancellation requests will not be accepted.

Annual Memberships: Annual membership fees are non-refundable. If you wish to discontinue services prior to the end of your annual term, you will not be entitled to a refund of any remaining prepaid fees.

Appointment Cancellations: We request that you cancel or reschedule appointments with at least 24 hours' notice. Late cancellations or no-shows may be subject to a cancellation fee.

Refund Policy: Except as expressly stated otherwise, all fees paid are non-refundable. If you believe you have been charged in error, please contact us within 30 days of the charge at info@e-hwc.com.

Prescriptions & Medications

Prescriptions issued through eHormone & Wellness Center are subject to the following terms:

• Prescriptions are issued solely at the discretion of our licensed providers following a proper medical evaluation
• We work with licensed compounding pharmacies and may prescribe FDA-approved brand-name medications where appropriate
• Compounded medications are not FDA-approved as finished drug products; your provider will discuss the benefits and risks with you
• Prescription refills require ongoing membership and compliance with follow-up visits and lab requirements as directed by your provider
• You are responsible for understanding and following all instructions provided with your medications
• Medications are prescribed for your personal use only and must not be shared with others
• We are not responsible for delays, errors, or issues arising from third-party pharmacy or shipping services

User Conduct

By using our website and services, you agree not to:

• Provide false, misleading, or fraudulent information at any time
• Use our services for any unlawful purpose or in violation of any applicable laws
• Harass, threaten, or abuse our staff, providers, or other users
• Attempt to gain unauthorized access to our systems, accounts, or data
• Interfere with or disrupt the integrity or performance of our website or services
• Use automated tools to scrape, collect, or access our website content without authorization
• Reproduce, distribute, or exploit our website content, trademarks, or materials without prior written permission
• Misrepresent your identity or impersonate any person or entity

We reserve the right to terminate access to our services immediately and without notice for violations of this section.

Intellectual Property

All content on our website — including text, graphics, logos, images, and software — is the property of IHC LLC DBA eHormone & Wellness Center or its content suppliers and is protected by applicable intellectual property laws.

You are granted a limited, non-exclusive, non-transferable license to access and use the website for personal, non-commercial purposes only. • Warranties that the website will be uninterrupted, error-free, or free of viruses or harmful components
• Warranties regarding the accuracy, reliability, or completeness of any information on the website

We do not warrant or guarantee specific health outcomes. Individual results from our services will vary based on a wide range of factors including individual health conditions, compliance with treatment protocols, and other variables outside our control.

Limitation of Liability

To the fullest extent permitted by applicable law, IHC LLC DBA eHormone & Wellness Center, its officers, directors, employees, agents, affiliates, and licensors shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to:

• Loss of profits, revenue, data, goodwill, or other intangible losses
• Damages arising from your access to, use of, or inability to access or use our services
• Damages arising from unauthorized access to or alteration of your transmissions or data

In no event shall our total cumulative liability to you for all claims arising out of or relating to these Terms or our services exceed the greater of (a) the total fees paid by you to us in the six (6) months preceding the claim, or (b) $500.

Some jurisdictions do not allow the exclusion or limitation of certain warranties or liabilities. In such jurisdictions, our liability is limited to the maximum extent permitted by law.

Indemnification

You agree to indemnify, defend, and hold harmless IHC LLC DBA eHormone & Wellness Center and its officers, directors, employees, agents, and affiliates from and against any claims, liabilities, damages, losses, costs, and expenses (including reasonable attorneys' fees) arising out of or in any way connected with:

• Your access to or use of our website or services
• Your violation of these Terms
• Your violation of any applicable law or the rights of any third party
• Any false, inaccurate, or misleading information you provide to us

Governing Law & Dispute Resolution

These Terms shall be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of law provisions.

Any dispute, claim, or controversy arising out of or relating to these Terms or your use of our services shall first be addressed through good-faith negotiation between the parties. If the parties are unable to resolve the dispute through negotiation, the dispute shall be submitted to binding arbitration in accordance with the rules of the American Arbitration Association, conducted in Dallas County, Texas.

Notwithstanding the foregoing, either party may seek injunctive or other equitable relief in any court of competent jurisdiction to prevent irreparable harm. You agree to waive your right to participate in any class action lawsuit or class-wide arbitration.

Termination

We may terminate or suspend your access to our services immediately, without prior notice or liability, for any reason including if you:

• Breach any provision of these Terms
• Provide false or fraudulent information
• Engage in abusive, threatening, or disrespectful conduct toward our staff
• Fail to pay amounts owed in a timely manner
• Engage in any conduct that we determine, in our sole discretion, is harmful to us, other patients, or third parties

Upon termination, your right to use our services will cease immediately. Provisions of these Terms that by their nature should survive termination — including disclaimers, limitations of liability, and indemnification — will survive termination.

Changes to These Terms

We reserve the right to modify these Terms at any time. When we make material changes, we will update the "Last Updated" date at the top of this page. For significant changes, we may also notify you by email or through a prominent notice on our website.

Your continued use of our website or services after any changes to these Terms constitutes your acceptance of the revised Terms. If you do not agree to the updated Terms, you must discontinue use of our services.
